| Please download the appropriate form, fill it out and return it with your sample. |
| For column screening, please submit at least 25 mg of sample along with the Column Screening Questionnaire. |

For preparative separations, please submit at least 200 mg of sample along with the Preparative Application Questionnaire.
When sending in your material for preparative separation, please include the Preparative Material Submission Form.
